In Internal Medicine will begin to
charge a $25.00 fee for patients who
do not provide cancellation notice
for his/her scheduled appointment.
We ask that you notify us anytime prior to
your appointment to avoid this fee.
Fees - Fees are based on how comprehensive
your visit is. There may be additional
charges for any lab work that your
physician may order. A lab designated
by your insurance company will
bill you separately in most cases.
Below is a listing of fees.

Payment - We consider payment of services
to be the responsibility of the
patient in physician-patient relationship.
Therefore, we would like to share
with you our expectations for payment.
We
participate with various
insurance companies and managed
care plans. We will file with your
insurance company on your behalf,
less any co-payments, deductibles,
and non-covered benefits which
are expected at time of service.
There
are several commercial insurance
plans that we do not actively participate
with. If you participate with a commercial
insurance plan, we ask that a full
payment be made at the time of service.
As a courtesy, we will then submit
your claim on your behalf, requesting
that full payment be remitted to
you. If you are unable to make a
full payment, we will be unable to
provide this courtesy to you.
Payment is expected at time of
service. Partners In Internal
Medicine accepts cash, check,
Visa, and Mastercard, in
addition to setting up payment
arrangements.
Please
note that payment arrangements
can be set up at anytime, regardless
of insurance, by speaking with a
billing service representative. They
can assist you with payment options
that meet your needs. |
